That a big dilemma, bargaindoctor. You have a lot of time, money and energy invested in being a doctor at this point and I can understand why that would be hard to walk away from.

It may sound silly, but one of the best decision making tools can be Rock, Paper, Scissors (http://www.blog.neverthesamerivertwice.com/2007/08/01/sliding-scales-change-vs-accept-it-as-it-is/). When you are playing the game, be really attentive to how you feel about the outcome. If you are happy, go with the choice. If you are sick to your stomach, do the opposite.

I do the same! Though I flip a coin. I say, "Heads means I stay a doctor for now. Tails means I quit right away and do something else." Then I flip the coin and if I'm happy with the result then I know that's what I wanted; and if I'm unhappy then I know that I had wanted the opposite. Either way, I then ignore the damn coin and do what I just found out that I had wanted all along.

Why not come up with a muse that utilizes your medical skills? Even better, a muse around something you love to do, and use some of your new free time to help people? It sounds like you are always going to be part doctor, doesn't sounds like a black & white issue to me...
